Anthem Blue Cross PPO

The IBT provides major medical insurance for those PORAC members whose employers have contracted with CalPERS for such coverage. Anthem Blue Cross administers the IBT’s plan. The benefits are offered through an 80/20 PPO plan for the Basic/Classic, Out-of-State and Supplement to Medicare Plans.

If you’re not in the PORAC sponsored health plan, consider making the switch to the PORAC plan during the next open enrollment period.

How do I change my address? Top

Address changes must be submitted in writing to the IBT. Please complete the Change of Address form and return to the IBT. A completed form can be returned via one of the following methods: mail to IBT of PORAC, 2960 Advantage Way, Sacramento, CA 95834; fax to (916) 999-8892; or email to rgonzalez@ibtofporac.org.

You must also contact CalPERS at (888) 225-7377 to ensure your address is updated in their system. CalPERS will not accept third party requests for address changes. Once the address has been updated with CalPERS, they will submit the request for the address to be updated in Anthem’s system.

How do I make changes to my health plan? Top

To make any changes to your health plan, including adding and/or removing dependents, actively working members should contact their human resources (HR) or personnel department to request changes. Retirees must contact CalPERS directly at (888) 225-7377.

Note: Any additions or deletions of dependents must be done within 30 days of the date of change.

How do I request new medical cards? Top

New medical cards can be requested by calling the IBT of PORAC at (800) 655-6397.

How do I register online with Anthem Blue Cross? Top

To register with Anthem Blue Cross, go to Anthem.com and click the Log In button on the top right corner of the web page. You will be redirected to the Log In page. You will need to select the Register Now link below the Log In button. Follow the steps provided to complete your registration.

I am retired, how do I maintain my Anthem Blue Cross health plan sponsored by the Insurance and Benefits Trust (IBT) of PORAC? Top

Retirees or soon to be retired, already enrolled in the health plan must apply for Retired Associate Membership (RAM) with the Peace Officers Research Association of California (PORAC) membership department. Click here for the RAM IBT application.

I am not currently enrolled in the Anthem Blue Cross health plan sponsored by the Insurance and Benefits Trust, but was a member of PORAC. How do I enroll in the Anthem Blue Cross health plan sponsored by IBT? Top

The health plan sponsored by the IBT is offered thru CalPERS and therefore requires that your employer be a contracting agency by way of a PEHMCA participation agreement. If you are an actively at work employee, please see your human resources (HR) or personnel department to confirm PEHMCA participation and obtain health plan option information. If you are a retiree desiring to enroll in the Anthem Blue Cross health plan sponsored by the IBT, you must first make sure your prior employer is PEHMCA approved and that you meet the eligibility requirements as set forth by the Eligibility Policy. I am turning 65 years of age, what should I do? Top

Medicare-eligible individuals turning age 65 must apply for Medicare three months before their 65th birthday. Please contact Social Security directly at (800) 772-1213 to determine if you qualify for Medicare. If you lack the sufficient required Medicare quarters (40) you can contact CalPERS at (888) 225-7377 and they will assist you in filing the necessary forms to remain in the Basic Plan.

I already have my MediCare cards for medical, what do I do for prescriptions (Part D)? Top

If you are planning to keep the Anthem Blue Cross Plan sponsored by the Insurance and Benefits Trust (IBT) of PORAC as your supplemental coverage to Medicare, you don’t need to sign up for Part D with MediCare. However, to maintain your prescription coverage, you must complete the Anthem Blue Cross MedicareRx (PDP) form. You must also contact CalPERS to update your status to MediCare recipient. Please note that under no circumstance are you allowed to sign up with another Part D provider as it may lead to the cancellation of your health care coverage thru CalPERS!

How do I make changes to my dental and vision plan? Top

Active members must contact their human resources (HR) or personnel department.

Retired members enrolled in the Delta Dental and VSP plans sponsored by the Insurance and Benefits Trust (IBT) of PORAC must send in a written request to Insurance Services, IBT of PORAC, 2960 Advantage Way, Sacramento, California, 95834; via fax to (916) 999-8892; or by email to healthplan@ibtofporac.org.

I am a retiree. How do I cancel my dental and vision plan? Top

My provider told me I need to submit the claim to Anthem myself, how do I do that? Top

Health claims may be submitted online via Anthem Blue Cross’s website at www.anthem.com and by logging onto your account. If you live outside of California, be sure to follow instructions on where to submit your completed claim form.

Health claims may also be submitted by clicking here to obtain the claim submission form, printing / completing the form and mailing the form, along with the invoice from your provider, to the address on the form.
